Appointments

Academy Orthopaedic Associates, PC is open Monday to Friday by appointment only. When calling to schedule an appointment, please state the purpose of the appointment clearly to allow for the appropriate amount of time to be allotted in the schedule. If you need to schedule an emergency appointment, there are a number of daily slots reserved for this purpose. Please bring all insurance cards, referrals, x-rays and test results pertaining to present injury or condition. Any radiology studies left in the office after your appointment will be discarded.

Every effort will be made to insure that our physicians keep close to schedule. Please remember that the nature of medicine assumes that certain unexpected and emergent situations will arise throughout the day. If you are a new patient, we ask that you arrive ten (10) minutes earlier than your scheduled appointment to complete the necessary paperwork.

Due to the rising cost of medical costs, if you are unable to keep your appointment, we require you to cancel twenty-four hours (24) in advance of your scheduled time. If you do not cancel within twenty-four (24) hours, there will be a $25.00 fee, payable before your next scheduled appointment.

Insurance and Payment Information

We accept most medical insurance plans and have affordable rates and flexible payment plans for individuals without health insurance. Please check with your insurance carrier or contact our billing department to see if your insurance plan is accepted. Payment Plans are arranged by contacting our billing department as well.

Referrals

Please note that it is your responsibility to understand your insurance plan requirements. If you have an HMO or other managed care plan, and are required to obtain a referral, you must have it available on the date of your appointment. If a referral is not available (via electronically or hardcopy), your appointment will be rescheduled.

Co-Pays and Payment Options

The law requires us to collect all co-payments and deductibles. Co-pays are collected at the time of service. If we do not collect these payments, we are violating our contract with your insurance company. Methods of payment accepted are Cash, Check, Visa/Mastercard and Debit.

Telephone Messages

The office and phone lines are open from 8:30am on weekdays, and our reception staff handles all of our calls. Please understand that we see patients five (5) days a week, and the physician may be with other patients, and therefore, not immediately available to provide an answer to your questions or return your call personally. All patient calls are addressed before the physicians leave on the same business day.

Disability Forms

We will be happy to complete all the appropriate and necessary disability forms that you have. A fee of $10.00 for each form must be paid prior to pick up or mailing of forms. All forms will be completed within three (3) business days. Our staff will contact you upon completion of the forms.

Prescriptions & Renewals

Please request all prescriptions and authorizations for renewals between 8:00am – 5:00pm, Monday to Friday, when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances.
